1. All trips must be part of the instructional program and therefore included in the teacher's approved plans. 

2. The specified trip must be planned and approved by the administrator/principal three weeks before scheduled trip date. 

3. This form must be filled out and approved by the administrator/principal before discussing the trip with the class.

4. All guidelines must be followed according to Nebo Crossing Academy Guidelines.
